Change can feel like a big deal, but when it means more choice, flexibility, and support designed just for you, it’s something to get excited about! The way aged care support is delivered in Australia is evolving, and here at New Horizons, we’re ready to guide you every step of the way.
For over 55 years, we’ve been dedicated to supporting people to live their best lives. As we look towards the new Support at Home program, our commitment to your wellbeing is stronger than ever. Let’s explore what these positive changes mean for you.
Currently, you might be familiar with the Home Care Package (HCP). From 1 November 2025, this program will be called Support at Home.
The goal is simple: to give you more choice and control over your care. The new system is designed to be more personal, flexible, and easier to navigate. It will focus on your individual needs and goals, creating a support plan that’s tailored specifically to you.
Whether you’re already receiving support from us or are just starting to think about your options, we want you to feel confident and informed.
Please be reassured, your support will continue. Your wellbeing is our absolute priority, and we will ensure a smooth and simple transition for you.
For Home Care Package (HCP) clients: You will automatically move over to the new Support at Home program. We’ll be in touch well before the changes happen to talk you through what your new plan will look like and answer all your questions. There will be no interruption to your services.
For Commonwealth Home Support Programme (CHSP) clients: If you are a CHSP client, nothing changes for you on the 1st of November. The Government has indicated changes will take effect to CHSP no earlier than July 2027.
Our promise is to make this process seamless, keeping you informed and supported at every stage.
This is a fantastic time to start planning for your future! The new Support at Home program will make it easier than ever to access the right support for your needs. Our friendly team can help you understand the process, from the first assessment to designing a personalised plan that helps you achieve your goals and maintain your independence at home.
With our deep experience in disability, mental health, and community support, we understand that great care is about more than just services – it’s about partnership, respect, and helping you live a fulfilling life.
We’re here to help you:
Understand the changes in simple, clear terms.
Navigate the new assessment process with confidence.
Design a support plan that makes the most of your individual budget.
Ensure a seamless transition to the new program with no stress.
We’re excited about these changes and the new possibilities they open up for you to live independently and well.
